St. Ann's Community in Irondequoit transforms the way care is delivered

A new, four-story Transitional Care Center is replacing the story The Heritage building, which is coming down later this summer.

The interiors of two new buildings, one each on St. Ann&#;s Community&#;s Irondequoit and Webster campuses, have been set up to be like homes.

And who wouldn&#;t like the granite countertops, state-of-the-art appliances, and flat screen TVs in their homes?

Angelina Romano of Irondequoit was having breakfast in the new Wegman Transitional Care Center on St. Ann&#;s Irondequoit campus, Portland Ave., earlier this month.

&#;I love it; I&#;m glad I came here,&#; Romano said.

Each of the three full floors of the new center in Irondequoit has two &#;households,&#; each with 12 private rooms, a parlor, great room and large kitchen.

Meals are no longer served on trays. Patients serve themselves from built-in steam tables &#; or nurses prepare them something else they might like to eat.

The new buildings in Irondequoit and Webster, both officially opened during Mother&#;s Day ceremonies and festivities Saturday, May 12, are part of an ongoing initiative to transform the way St. Ann&#;s Community delivers care to its residents and patients.

A total of patients was moved to new facilities in late March and mid-April, so that the story Heritage building on the Irondequoit campus is now empty.

Sometime this summer, the well known tower on the Irondequoit campus will be coming down.
&#;We do talk reverently about it (The Heritage); we know people called it home, spent their careers there, and died there,&#; said Michael McRae, senior vice president of nursing home operations.

&#;But, the story Heritage building, designed as a health-related facility, was no longer meeting the needs of seniors,&#; McRae said. &#;We took feedback we heard from seniors and incorporated it into our strategic plan.&#;

The new four-story building on Portland Avenue in Irondequoit houses the bed St. Ann&#;s Transitional Care Center for rehabilitation services, where residents recuperating from an illness or surgery are expected to stay only short-term, from a couple of days to no more than a few months.

The Irondequoit facility is complete with the bed Palliative Center for Caring (hospice) on the fourth floor.

At the Cherry Ridge campus on Ridge Road in Webster, a new three-story building features a bed transitional care unit and 60 long-term, skilled nursing care beds.

For the first time, the Webster campus now offers the complete continuum of care, from apartments and cottages for independent seniors to assisted, skilled and transitional care with rehabilitation services, McRae said.

&#;With more people aging in place and remaining in their own homes longer, what we heard from seniors is that they needed greater access to transitional care, rather than going directly from hospital to home,&#; McRae said. &#;This is the solution.&#;

He predicted that the average length of stay in the new transitional care center will be about 20 days. And, the center is not just for seniors.

The new households at the center were designed to be more &#;real&#; and less institutional, McRae said, noting that therapists will also be coming to work with patients in the households, rather than patients going to a main therapy gym.

The household or &#;country kitchen&#; concept is also coming to St. Ann&#;s Home, the skilled nursing facility on the Irondequoit campus, over the next year or so.

In the lobby of the new Irondequoit building is a Bistro Cafe, open to anyone, with wireless service.

&#;I&#;ve been in this business (senior care) for 24 years and I&#;ve never seen anything like this,&#; McRae said. &#;The changes have been done for all the right reasons and the outcome is spectacular.&#;
